What You’ll Do:
- Conduct home visits and provide compassionate symptom management
- Create, update, and execute individualized plans of care
- Partner with the interdisciplinary team to align patient goals and wishes
- Provide honest, supportive communication with families and caregivers
- Participate in team meetings and staff training to improve care delivery
Required Background:
- Active MN RN license
- Previous hospice, palliative, or oncology experience
- Strong patient advocacy and emotional resilience
- Ability to multitask, document precisely, and manage a full caseload
- Respect for cultural, religious, and personal diversity
