Health Readiness Resources (HRR) provides high-quality, person-centered telehealth services to individuals with Intellectual and Developmental Disabilities (IDD) and Traumatic Brain Injuries (TBI). Our integrated care model focuses on the physical, behavioral, and social well-being of each person—ensuring coordinated, compassionate, and accessible care across Alaska.
Position Overview
We are seeking a dedicated and empathetic Care Manager to join our Telehealth IDD Program. In this remote role, you will serve as a primary point of contact for individuals and families, helping to coordinate care, support transitions, and connect patients to essential health and community services. You’ll play a key role in promoting wellness, behavioral stabilization, and independence for those living with IDD and/or TBI.
Key Responsibilities
• Conduct person-centered assessments to evaluate physical, behavioral, and social health needs.
• Develop and manage individualized care and support plans.
• Coordinate services among medical, behavioral health, and community providers.
• Facilitate safe care transitions and ensure continuity of care.
• Serve as a consistent point of contact for patients, families, and care teams via telehealth.
• Educate individuals and families on wellness, self-advocacy, and available community resources.
• Monitor progress, adjust interventions as needed, and provide proactive and crisis support.
• Maintain accurate documentation and ensure compliance with state and program standards.
• Stay up to date with Alaska regulations, care management best practices, and telehealth advancements.
Qualifications
• Bachelor’s or Master’s degree in Nursing, Social Work, Behavioral Health, or a related field.
• Active Alaska licensure (RN, LCSW, LPC, LCPC, MHC, or equivalent) – Required.
• 1–2 years of experience in care coordination, case management, or direct clinical care.
• Experience with IDD, TBI, mental health, or complex care populations preferred.
• Telehealth or virtual care experience strongly preferred.
• Strong communication, organizational, and problem-solving skills.
Additional Requirements
• Must pass background and healthcare registry checks.
• Completion of all mandatory HRR trainings and orientation before onboarding.
Schedule Options
• Flexible scheduling: full-time, part-time, or contract.
• Day, evening, night, and weekend shifts available.
• On-call rotation participation as needed.
