Role Overview: IHS Designated Coordinator
The IHS Designated Coordinator plays a pivotal role in our Individualized Home Supports with Training (IHS) program. This position is responsible for meeting with, setting up, and providing ongoing training and support to individuals accessing our IHS program. The ideal candidate will have a strong background in human services and a passion for delivering high-quality support to persons with disabilities or those aged 65 and older.
Key Responsibilities:
- Meet with individuals to assess their needs and set up support plans tailored to their requirements.
- Provide ongoing training and support to individuals accessing the IHS program, ensuring they receive the highest quality of care.
- Travel to clients' homes in the surrounding Metro counties to deliver support services.
- Maintain accurate records and reports as required by the program and regulatory bodies.
- Collaborate with other professionals and agencies to ensure comprehensive support for individuals.
- Demonstrate creativity, critical thinking, and problem-solving skills in addressing the complex needs of individuals.
Requirements and Qualifications
To be considered for this role, candidates must meet one of the following educational and experience requirements:
- Bachelor's degree in a field related to Human Services, and one year of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older.
- Associate degree in a field related to Human Services, and two years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older.
- Diploma in a field related to Human Services from an accredited postsecondary institution and three years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older.
- Minimum of 50 hours of education and training related to Human Services and disabilities; and four years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Additional Requirements:
- 1 year of experience in a Human Service field.
- Excellent organizational, problem-solving, communication, and interpersonal skills.
- Ability to pass a background study with the Minnesota Department of Human Services.
- Valid driver's license, clean driving record, and reliable transportation for work purposes.
- Ability to travel independently across Accra's geographic service areas.
