About the Role:
As a compassionate and skilled Home Health Registered Nurse (PRN), you will have the opportunity to make a meaningful impact on the lives of patients and their families. Responsibilities:
- Coordinates and oversees all aspects of patients' home care, including developing and implementing care plans
- Collaborates with patient care teams to provide high-quality, patient-centered care
- Educates families on patient condition, needs, and care
- Performs physical assessments, checks vital signs, and manages medications
- Administers injections, IV therapy, or tube feedings, and maintains catheters
- Completes documentation timely and accurately, and communicates patient progress to the clinical manager, physician, and care team
- Supervises LPNs and HHAs, and participates in quality improvement initiatives
Qualifications:
- Current RN license, specific to the state(s) you are assigned to work
- Minimum one year of clinical experience as a registered nurse (RN), or completion of an RN Intern program
- Minimum six months of home health experience
- Current CPR certification
- Valid driver's license, reliable transportation, and liability insurance
- Able to assess patient status and identify requirements relative to age-specific needs
- Proficient in the use of computers, including navigation of computer desktop, email, and online HR services
