Responsibilities
As a Remote Clinical Case Manager, you will be a vital point of contact for patients, families, providers, and other stakeholders. You will:
- Coordinate patient care across the continuum of care, including referrals, home visits, and community resources.
- Assess patient needs and develop care plans to improve health outcomes and ensure access to benefits.
- Communicate with patients and partners to provide education, support, and advocacy.
- Maintain accurate and timely documentation of case management activities.
- Collaborate with a multidisciplinary team to optimize patient well-being.
- Ensure compliance with all applicable regulations and guidelines.
- Proactively address issues to improve patient outcomes and enhance the overall patient experience.
Qualifications
Required:
- Demonstrated ability to effectively communicate and build rapport with individuals and families.
- Strong organizational and time management skills.
- Ability to work independently and as part of a team in a remote environment.
- Reliable internet access and a dedicated workspace.
Preferred:
- Experience in clinical case management, care coordination, or home medical services.
- Bilingual proficiency in Spanish/English.
- Experience with Federal health insurance populations.
- Valid driver's license and reliable transportation for occasional in-person visits within a designated geographic area.
