As a Case Manager RN, you will play a vital role in ensuring the continuity of care for our patients, leveraging your nursing expertise to drive quality outcomes and improve patient experiences.
Key Responsibilities:
- Utilize your nursing knowledge and skills to facilitate continuum of care, ensuring appropriate resource utilization and patient quality outcomes.
- Prioritize patient care needs, addressing emerging issues and developing personalized care plans in collaboration with patients, families, and caregivers.
- Coordinate treatment plans with the care team, triaging interventions and using protocols and pathways to optimize clinical outcomes.
- Monitor and coach patients using motivational interviewing and behavioral change techniques to maximize self-management.
- Maintain effective communication with physicians, hospitalists, extended care facilities, patients, and families to ensure seamless care transitions.

Requirements:
- Active RN license in current state of residence, with the ability to qualify for additional state licenses as requested.
- 2+ years of nursing experience in a hospital, acute care, or direct care setting.
- Bilingual proficiency in Spanish, Cantonese, or Mandarin.
- Renal, Chronic Kidney Disease, or Dialysis Care experience.
- Computer proficiency, including MS Office and electronic health records.
Preferred Qualifications:
- BSN degree.
- Certified Case Manager (CCM) certification.
- Diabetic educator experience.
- ICU, Cardiology, or Critical Care experience.
- Telephonic case management experience.
