Position Overview
We're seeking tech-savvy, compassionate LPNs to conduct monthly telephonic outreach to a panel of chronically ill patients, developing and delivering personalized care plans that address their unique health concerns and challenges. As a Patient Care Coordinator, you'll work in partnership with healthcare systems and practices to provide these services, striving for care continuity and engagement. This is a full-time opportunity that requires 40 hours of work per week, between the hours of 8 am – 6 pm in the patient’s time zone, Monday – Friday.
Key Responsibilities:
- Manage and support patients' healthcare needs via virtual phone conversations, ensuring personalized care and attention.
- Conduct and document visits leveraging technology platforms and Electronic Health Records (EHRs), maintaining accurate and up-to-date patient information.
- Develop and implement personalized care plans to address patients' physical, mental, and preventative health needs, promoting holistic well-being.
- Virtually coach patients to reach their care goals by following medical treatment plans that include healthy eating, physical activity, stress reduction, and other lifestyle modifications.
- Connect patients with resources such as nutritionists, nurses, and doctors who can assist them in meeting their care needs, facilitating access to care.
- Prepare patients for appointments and procedures, and provide follow-up as needed, ensuring seamless care transitions.
- Coordinate administrative aspects of medical record recovery, appointment management, and insurance claims, demonstrating organizational and time management skills.
- Review insurance plans and suggest tips to get the most from their benefits, empowering patients to make informed healthcare decisions.
- Proactively suggest services or preventive screenings relevant to each patient, promoting preventive care and early intervention.
Requirements and Qualifications
Essential:
- Compact Nursing License that is clear and active.
- Reside and work in the continental US.
- Minimum of two years of clinical experience, with experience in care coordination or care management, health coaching, and motivational interviewing being highly desirable.
- Hands-on experience and comfort with Electronic Medical Records (EHRs).
- Proficiency in Microsoft Office products such as Teams, Outlook, and Excel.
Preferred:
- Experience in geriatrics, home health, or primary care.
- Certification in care coordination or a related field.
- Familiarity with healthcare technology platforms and telehealth services.
