Key Responsibilities:
- Conduct virtual phone conversations to manage patients' healthcare needs and develop tailored care plans addressing physical, mental, and preventative health challenges.
- Utilize cutting-edge technology platforms and Electronic Health Records (EHRs) to document patient visits and ensure seamless care coordination.
- Coach patients on treatment plans, including nutrition and wellness strategies, to promote holistic health and wellness.
- Connect patients with essential resources and prepare them for medical appointments to ensure comprehensive care.
Requirements:
- Active Compact LPN License
- Minimum 2 years of clinical experience, with care coordination experience preferred
- Proficiency in Electronic Medical Records and Microsoft Office
- Excellent communication and problem-solving skills
